Bridges of Wyoming provides Adult Day Services which are meaningful activities to the people we support. Adult Day Services ensure that a person maximizes his or her skills and abilities, participates in community activities, maintains their unique personal skills set, improves their relationships and builds on their strengths and core talents. Bridges of Wyoming provides person-centered supports to promote maximum independence.
Adult Day Services
Homemaker Services consist of general household activities such as meal preparation and routine household care. Bridges of Wyoming provides this service when the supported person is not able to care for themselves or manage their own household functions.
Residential Habilitation Services are unique person-centered supports that assist a person with acquiring, retaining and improving community living skills. Bridges of Wyoming provides these supports which include skills development, daily living activities, community integration, transportation, adult educational supports and social and leisure skill development.
This service offers assistance with acquisition, retention or improvement in self-help, socialization and adaptive skills. It also helps a person acquire new skills, work on acceptable and appropriate behavior, gain independence, network in their community and meet new friends. volunteer activities, libraries, cultural, religious or art centers and build and maintain social connections.
Individual Habilitation Training is a specialized 1:1 intensive training service to assist a person to acquire or improve skills; skills that will promote individual independence and a greater level of functioning. Bridges of Wyoming Direct Support Professionals may provide this service to a person who lives with an unpaid caregiver or a person who needs less than 24-hours of paid supervision and support.
Respite includes assistance with daily living activities, medication assistance and individual supervision. Respite cannot be used for childcare or daycare purposes while the primary caregiver is working and it cannot be used during services that are available through normal education activities, after school supervision, daytime services when the school is not in session or services to preschool age children.
Bridges of Wyoming provides supervision, socialization and assistance to a person so that they may gain independence in their home or community.
Personal Care Services assist people with disabilities to accomplish tasks that they would normally do for themselves if they did not have a disability. Assistance may be hands-on and Bridges of Wyoming Direct Support Professionals may actually perform a task for the person or direct a prompt to the person to perform a task. Personal Care Services may be provided periodically or on a continuing basis. Services may also include medication assistance or administration and range of motion exercises.
Personal Care Services
Bridges of Wyoming provides Supported Living Services which are person-centered supports to help a person acquire, retain or improve his or her community living skills. Supported Living services assist a person who lives in their own home and when the person requires a a variety of community based supports to live with a maximum level of individual independence.
